Spectrum Health’s Physician Liaisons were meeting with Referring Physicians (RP) with a variety of tools that were often outdated, redundant, and primarily developed for patients. What they really needed was a single tool that convinced the RPs that Spectrum Health could provide the best outcome for their patients and reassure them that their patients would be kept close to home and returned to them in good heart health. This print piece was developed with the Referring Physician in mind, but also included helpful evaluative components that encourage discussion between physician and patient – reassuring both parties of their decision to partner with the Spectrum Health Advanced Heart Failure Program.
